Forklift Safety Inspection Checklist: Daily Items Operators Should Check

A daily forklift inspection is required by Cal/OSHA before every shift — but many operators skip items or don't know what to check. Here's a complete daily inspection checklist.

A daily forklift inspection is one of the most important safety practices in any warehouse — and it's required by Cal/OSHA. Before operating a powered industrial truck, the operator must inspect it for safety hazards and document the inspection. Skipping the inspection or not documenting it can result in OSHA citations, equipment failures, and serious accidents.

This checklist provides a comprehensive guide to the daily items operators should check before every shift, for electric, propane, and diesel forklifts.

Why Daily Inspections Are Required

Cal/OSHA Title 8 CCR 3664(a) requires that "industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle."

The daily inspection serves several purposes:

  • Identify safety hazards before the forklift is operated — defective brakes, damaged forks, or leaking hydraulics can cause accidents
  • Prevent equipment damage — catching minor issues early prevents them from becoming major, expensive repairs
  • Provide documentation for OSHA compliance — if an accident occurs, the daily inspection log shows that the forklift was in safe operating condition at the start of the shift
  • Protect the operator — driving a forklift with known defects puts the operator and everyone around them at risk

The inspection must be documented — a written or digital record that the inspection was performed and what was found. An inspection that isn't documented is considered not to have been performed, from a compliance standpoint.

Pre-Shift Inspection Checklist: All Forklift Types

The following items should be checked on every forklift, regardless of fuel type:

1. Visual Walk-Around Inspection

Before getting on the forklift, walk around it and check:

  • [ ] Tires — check for wear, cuts, chunking, embedded debris, or improper pressure (pneumatic tires). Solid/cushion tires should be checked for chunking and wear patterns.
  • [ ] Wheels and lug nuts — check that all wheels are secure and lug nuts are tight (if applicable)
  • [ ] Forks — check for cracks, bends, excessive wear, and proper positioning. Forks should be evenly spaced and secured.
  • [ ] Fork tips — check that the fork tips are not worn excessively or damaged. Worn tips make it difficult to enter pallets and can cause loads to slip.
  • [ ] Mast — check for visible damage, cracked welds, bent channels, or hydraulic leaks
  • [ ] Chains — check for proper tension, wear, stretch, and lubrication. Chains should not have kinks, rust, or broken links.
  • [ ] Hydraulic hoses and fittings — check for leaks, cracks, bulges, chafing, and loose fittings
  • [ ] Cylinders — check for leaks at seals, dents, or damage to the cylinder rods
  • [ ] Overhead guard — check for cracks, bent or damaged members, and secure mounting
  • [ ] Load backrest — check for damage and secure mounting
  • [ ] Body and chassis — check for damage, loose panels, or missing guards
  • [ ] Data plate — verify that the capacity/rating plate is present, legible, and matches the forklift configuration
  • [ ] Seat belt or restraint system — check that the seat belt is present, functional, and not damaged
  • [ ] Operator compartment — check for debris, oil on the floor, or damaged controls
  • [ ] Engine/motor compartment — check for debris, leaks, or unusual odors (do not open compartments with the engine running)

2. Operational Checks

After the visual inspection, start the forklift and check:

  • [ ] Horn — test the horn and verify it sounds
  • [ ] Lights — test headlights, taillights, and strobe lights (if equipped)
  • [ ] Backup alarm — verify the backup alarm sounds when the forklift is placed in reverse
  • [ ] Seat belt — verify the seat belt latches and unlatches properly (if equipped)
  • [ ] Gauges — check that all gauges read within normal range (fuel, battery, temperature, hour meter)
  • [ ] Brakes — test the service brake and parking brake. The forklift should stop smoothly and the parking brake should hold on an incline.
  • [ ] Steering — check that the steering operates smoothly with no excessive play or binding
  • [ ] Lift mechanism — raise and lower the forks through the full range of motion. Check for smooth operation, unusual noises, or drift.
  • [ ] Tilt mechanism — tilt the mast forward and backward through the full range. Check for smooth operation, unusual noises, or drift.
  • [ ] Side shift (if equipped) — test the side shift through its full range
  • [ ] Hydraulic controls — test all hydraulic functions (attachment controls, if equipped)
  • [ ] Accelerator — check that the accelerator responds smoothly (if applicable)
  • [ ] Transmission — check that the transmission shifts smoothly between forward and reverse (if applicable)
  • [ ] Hour meter — record the hour meter reading for maintenance tracking

3. Fuel-Specific Checks

Electric Forklift Additional Checks

  • [ ] Battery charge level — verify the battery has sufficient charge for the shift
  • [ ] Battery cables — check that cables are tight, clean, and not corroded or damaged
  • [ ] Battery connectors — check for damage, melted, or burned contacts
  • [ ] Electrolyte level — check battery water level (if the battery has removable caps). Add distilled water if low. (This check may be done less frequently — weekly or monthly — depending on the battery and usage.)
  • [ ] Battery case — check for cracks, swelling, or leaks
  • [ ] Charger connector — check for damage or corrosion
  • [ ] Controller display — check for fault codes or warning indicators

Propane Forklift Additional Checks

  • [ ] Propane tank level — check the tank gauge or feel the tank weight to estimate fuel level
  • [ ] Tank valve — verify the tank valve is fully open
  • [ ] Fuel line — check the fuel line for leaks (use a soap solution if a leak is suspected)
  • [ ] Tank mounting — verify the tank is securely mounted and the strap or bracket is tight
  • [ ] Regulator — check for frost, damage, or leaks
  • [ ] Engine oil level — check the engine oil dipstick and add oil if low
  • [ ] Coolant level — check the coolant level in the radiator or overflow tank
  • [ ] Air filter — check the air filter condition (visually inspect, replace if dirty)
  • [ ] Fan belt — check the fan belt for tension, cracks, or fraying
  • [ ] Exhaust system — check for leaks, damage, or excessive noise
  • [ ] Engine start — start the engine and check for smooth idle, unusual noises, or excessive smoke

Diesel Forklift Additional Checks

  • [ ] Fuel level — check the diesel fuel gauge
  • [ ] Engine oil level — check the engine oil dipstick and add oil if low
  • [ ] Coolant level — check the coolant level in the radiator or overflow tank
  • [ ] Fuel filter/water separator — check for water in the separator and drain if necessary
  • [ ] Air filter — check the air filter condition
  • [ ] Fan belt — check the fan belt for tension, cracks, or fraying
  • [ ] Glow plugs — verify the glow plug indicator illuminates (if equipped)
  • [ ] Exhaust system — check for leaks, damage, or excessive smoke
  • [ ] Engine start — start the engine, allow it to warm up, and check for smooth idle, unusual noises, or excessive smoke

What to Do If Defects Are Found

If any defect is found during the pre-shift inspection, the operator must:

  1. Not operate the forklift — if the defect affects safety, the forklift must not be placed in service
  2. Tag the forklift out of service — use a lockout/tagout tag or other visible indicator to mark the forklift as out of service
  3. Report the defect — notify the supervisor or maintenance department
  4. Document the defect — note the defect on the daily inspection log, including the specific issue, date, and operator name
  5. Do not attempt repairs unless the operator is qualified to do so
  6. Use a different forklift — if available, the operator should use a different, inspected forklift to complete their tasks

What Defects Require Tagging Out?

Any defect that affects safety should result in the forklift being tagged out, including:

  • Brake failure or reduced braking performance
  • Steering problems — excessive play, binding, or failure to steer
  • Hydraulic leaks — especially leaks that affect lifting, tilting, or steering
  • Damaged forks — cracks, bends, or excessive wear
  • Damaged overhead guard or load backrest
  • Missing or non-functional safety devices — horn, lights, backup alarm, seat belt
  • Tire damage — chunking, cuts, or separation
  • Fuel system leaks — propane, diesel, or battery acid leaks
  • Unusual noises — from the engine, transmission, or hydraulic system
  • Fault codes — on electric forklifts, any displayed fault code should be investigated
  • Damaged chains — kinks, rust, broken links, or excessive stretch
  • Damaged data plate — if the capacity plate is missing or illegible

Minor defects that don't affect safety (e.g., a scratched paint job, a loose non-essential panel) should still be reported but don't necessarily require the forklift to be tagged out.

Documentation Requirements

The daily inspection must be documented. The documentation should include:

  • Date of the inspection
  • Operator name
  • Forklift identification — serial number, asset number, or other identifier
  • Hour meter reading at the time of inspection
  • Items checked — the inspection checklist (can be a standard form with check boxes)
  • Defects found — if any, including the specific defect and whether the forklift was tagged out
  • Operator signature — acknowledging that the inspection was performed
  • Supervisor signature — if defects were found and reported

The inspection log should be retained for at least one year (Cal/OSHA can request past records during an inspection). Many facilities retain logs for longer periods as part of their maintenance records.

Common Inspection Mistakes

  • Rushing through the inspection — operators may skip items or not check thoroughly because they're pressured to start work
  • Not documenting the inspection — an undocumented inspection is considered not to have been performed
  • Filling out the form without actually inspecting — "pencil whipping" the inspection form is a common and dangerous practice
  • Not reporting defects — operators may fear that reporting defects will result in the forklift being unavailable, so they don't report minor issues
  • Operating with known defects — using a forklift with a known safety issue because a replacement isn't available
  • Not checking fuel-specific items — electric operators may not check battery cables, propane operators may not check the fuel line
  • Not checking the data plate — the capacity plate is critical for safe operation; if it's missing or illegible, the forklift should not be operated
  • Not checking the seat belt — the seat belt is a critical safety device; if it's missing or damaged, the forklift should not be operated
